I.    ACTIVITIES:

This month of March has been another busy month for us in the RHU. The number of activities that we had are as follows:

March 2, 2012 -   As per LGU routine, we had our 1st Friday Mass held at the Milaor  Covered Court as officiated by  the  Parish Priest, Rev.Fr. Ruben Buena. It is the day that we  have Epilepsy Patient’s Check-Ups in the RHU.  Residents of Milaor that have Epilepsy were seen by Doctors specialized in Epilepsy. I assisted one patient that was having seizure attack while the mass was on going. She has cough and colds that is why she is more likely to have an attack. I was given instruction by Dra.Llaguno to give her a glass of water after 30 mins. Then we observed the patient and took her vital signs. She was then prescribed her medications and was ordered to go home immediately.

March 5, 2012 – I was an emcee in the STI/ HIV-AIDS Awareness Seminar conducted by the RHU to the RSW (Registered Sex Worker) of the 3 Night Clubs located in the Municipality. Held at Sangguniang Bayan Hall. The goal is to disseminate information regarding STI’s to people who’s job uses sexual activity and to prevent getting the STI and HIV-AIDS. Mayor Rogelio Flores, V-Mayor Andre Hidalgo, DOH Rep Josephine Batalla, Maam Glenda Lucena, Maam Fe Jacar, Maam Mary Shienna Pecenio were present in the said seminar and they gave short messages / lecture that aid in the success of the seminar.

March 7, 2012 – after the daily routine of admitting patients and consultation, Maam Glenda Lucena refreshed the RHU Staff regarding Micronutrients as per her  training at Bicol Sanitarium, Cabusao, Camarines Sur. We were reminded of Vitamin A, Folic Acid, Iodine etc and how it is helpful in making a person healthy.

Baranggay Nutrition Scholar (BNS) conducted their monthly meeting regarding  Garantisadong Pambata (GP) Program of the DOH. Vitamin A supplement and Albendazole used for deworming to children ages 0-72 months old of Milaor was discussed.

March 12- 14, 2012 – 4P’s meeting / seminar at the Milaor Covered Court. The 3 day approving and processing of  credentials were participated by the different brgys of the town. Some of the brgys already claimed their allowance and was happy that they have aid in their everyday expenses.


March 13, 2012 – Mother’s Pre-Natal Check Up in the RHU. Again I injected tetanus toxoid vaccine to women that is still not complete  in the immunization. We have RHU Midwife that conducts Leopold Maneuver to each  pregnant woman and determines if the presentation of the baby is  normal according to its age of gestation. We give FeSo4 prenatal vitamins for  baby’s brain development inside the mother.

March 16, 2012 – The RN Heals Batch 3 Nurses started their duty in the RHU. They were Daisy Austria, Lorie Ann Cabase and John Leo Callo, all residents of Milaor.

NCF Students also conducted their duty at the  RHU on March 5,6,7 and 12,13,14, 2012. Helped in admitting patients, getting vital signs and compiling records.

March 27, 2012 – Liga ng mga Barangay Basketball Sports Competition started at Milaor Covered Court from 2:00 in the afternoon until 9:00 in the evening and we were shifted to be the Medical Team during each game.

March 28, 2012 – PHEMAP (public health emergency management plan) lecture was given to us by Mrs. Fe Jacar, our Sanitary Inspector and Ms. Jhoanna Marie Guerrero, RHU Nurse I emphasizing what transpired in their Seminar at Bicol Sanitarium was disseminated to us RHU-Staff.





March 30, 2012 – Blood Letting Program at the Milaor Covered Court. I took the vital signs of donors from all barangays of the municipality then they were interviewed by the BMC Staff. Next is blood typing and other blood laboratory tests before being extracted. After blood letting, they were given free snacks and lunch. They will also be notified if the blood that they gave is qualified or not to be transfused.

II.    LEARNINGS/INSIGHTS:
                 
                      I become updated of the current statistics of the people who are suffering from HIV/AIDS in the whole province of Camarines Sur and the whole Bicol Region. I become aware that the number of people having STI’s are increasing each year and thus the alarming situation in the country. As a Health Representative, it is my duty to inform people of this situation and help spreading correct information in preventing such cases and to encourage people that are sexually promiscuous or having profession that puts them at high risk to get tests for early detection and management.

                    I was refreshed in the use and role of the common micronutrients needed by the body. I was further educated what type of foods we can get these from.

                  During Mother’s Class, I am being trained to explain the topics more in our native dialect for the audience to easily understand the lessons we are incorporating in them.

                 PHEMAP suggested us to plan carefully how to manage disasters or emergencies that we may encounter in the municipality. It helps to plan so that we can prepare and make ways on how to prevent casualties during flood, typhoons, earthquakes, outbreaks, etc.

                    Blood letting activity helped in integrating useful benefits to a person’s health.

III. ISSUES/CONCERNS:

            The women that attended our mother’s class helped me realized that there are a lot of uneducated / uninformed mothers of what to do during their pregnancy.  Most of them are not aware that they are at risk when they give birth to non-birthing facility such as their homes. I was also shock to learn that most of them are not breastfeeding their babies although they have all the reasons to do so. They are not working mothers, they do not leave their babies at home, they don’t have any illness or disease that is contraindicated to breastfeed, etc. they just don’t have the initiative, trust & confidence in themselves that they can breastfeed. Some don’t also prepare complimentary feeding to their 6 mos. old children. They don’t even have prenatal check ups or even post natal check ups. They don’t complete their child’s immunization. They don’t go to health centers to avail of health services.

IV. RECOMMENDATIONS:

                  I strongly recommend to the women that attended the mother’s class that whatever knowledge that they gained in the mother’s class be put into action and practice them everyday in their life. Advocating proper information and education regarding health practices is highly emphasized in every mother’s class. Take advantage of every opportunity to educate patients, clients seeking help in the center needs health teaching too especially in taking medicines. They don’t have to self medicate. Entertain questions from them. Clarify issues or problems with them. Investigate reasons why they are not able to follow simple medical advice and supply an alternative way on how they can solve it. They should always be open to new medical advancement. Have an open mind and willingness to learn more.

I.        ACTIVITIES:

January is a very busy month, just right after the new year we had lots of activities in our Rural Health Unit. These are the ff:



I rewrote the directory of RHU in a new Log Book last Jan.2, 2012

ANLENE sponsored a FREE BONE SCAN in cooperation with the Local Government Unit of Milaor and full support of the Municipal Health Unit. This procedure usually costs 4k.

    Held at the Municipal Covered Court last January 4, 2012.

        After our right ankle is scanned, they give free taste.


I found out I need to drink more milk and less caffeine. I’m borderline to low risk and medium risk. This bone scan is to determine the risk for osteoporosis. If you are 25 yrs.old and above, you can check your bone density. We had this activity the whole day.


Last Jan.12, 2012 USANA Network Marketing Team came to our RHU and gave a business presentation of their products.

Every weekday afternoons, we are organizing the records in their alphabetical order. Then we put it back in their own respective family folders.

I was also tasked to paste the photos of the RHU Documentation to be passed to the Philhealth Office.
 





We visited our patient who has PTB inside the Provincial Jail. This patient is a resident of Milaor. The LGU-RHU is the one who admitted this patient for PTB medication. We endorsed 2months supply of intensive medication to the Jail Nursing Aid. We will supply the next batch of medication after 2 months. We educated the Nursing Aid regarding the medication schedule and other necessary information that is a “must know” of the patient and the other people within his surroundings.

 II.     LEARNINGS/INSIGHTS:

         This month I was able to develop a schedule to alphabetically arrange the patient’s records and at the same time still doing my daily routine. I was able to manage and budget my time inside the RHU so that I will be more efficient. I was able to give time to other tasks assigned to me by our Head Nurse. I helped in compiling records in accordance to Morbidity in every Barangays and Mortality Rate. We based our schedule to have Mother’s Class in Barangays that have a high Maternal Mortality Rate.


 III.   ISSUES/CONCERNS:

 
The numbers I made

          First: I made numbers 1-50 for the patients who are coming in. In the first week, 5 numbers went missing. It’s either the patient brought it home or they misplaced it. I gave it to the patients by order and in turn they give me their records so that I can put their initial vital signs and clinical complaint there. Then their numbers will be called in order for them to be seen by our Municipal Health Officer. Once they had been given primary treatment and prescriptions they will get their record back and thus give the number back. But not everyone in the center or the patients understood the process.
          Second: Most of them are not even bringing their record upon consultation. We had to make a new record and thus result to duplication and are most likely misplaced by the client again and again.
        Third: As per the Municipality of Milaor’s counterpart for the DOH RN Heals agreement, specifically “Joint Memorandum Circular No. 2011-0044 between the DILG, DOH and DSWD”, we did not receive anything yet since we started last November. Midwives assigned in other municipalities were already given by their LGU’s. We already followed up at our HR but we were endorsed to our Head Nurse which is Glenda T. LuceƱa. That our Head Nurse should be the one to talk to our Municipal Mayor.
         Fourth: Our Monthly Stipend. We were not advised any specific date when we will receive it. We don’t even know if it’s already in our ATM or not unless we follow up from our DOH Rep.


IV.   RECOMMENDATIONS:

        What we did, upon arrival of patients, we educate them not to bring the numbers home after their consultation. We made sure they understood the process. For the missing numbers, I needed to reprint and re-laminate it.
       With regards to the second concern, we came up of an idea. To those patients who didn’t bring their records and are members of 4ps, we warned them verbally that their records are legal documents needed to claim and prove that they are qualified benefactors of the 4ps fund.
Other centers are strict enough that without record, they won’t be entertained. We told them that this would be the last that we will give consideration, next time we will now implement the “No record, no check up Policy”. I hope this verbal warning will give results next month and always.
      For the third issue: I suggest that our Head Nurse and DOH Rep. go together to our Municipal Mayor and follow up in the status of our Allowance from LGU. If there is going to be an allowance or not, we should know.
        Fourth Issue, There should be a communication or initiative from our DOH Rep to inform us when it will be available. Because before we receive it, we already ran out of daily budget. That is to set up our expectation for us to be able to avoid getting frustrated. It is just upsetting that we are depending on our monthly stipend and we waited a month but its still not yet available. Practically speaking, we all need it to finance our daily necessities.

I.       ACTIVITIES:

          I started the shift for the month by logging in and out at our Daily Log Book at the Municipal Building. For every 1st Friday of the month, there is a Holy Mass held in the LGU- Milaor.  We have our flag ceremonies and flag retreats during weekdays. Weekends are considered our off. If it’s a national holiday, we don’t have any work.
           
          Our daily routine is to take vital signs of patients, getting records before our MHO prescribe any meds, give medicines if available as prescribed, refer to tertiary hospital if the patient’s case needs more intensive care or treatment, assist in conducting pap smear, more so, take charge of the facilitation of initial treatment to patients coming in.
           
During Wednesdays we help in the EPI for newborns and babies under 5 yrs. old.
This month, I helped facilitated the Sr. Citizens Anti-Flu Vaccination, another program of the DOH.



          For the month’s 2nd Tuesday we have a scheduled Pre-Natal Check ups for pregnant women. I administered Tetanus-Toxoid Vaccines to almost 50 pregnant women from our municipality.
            In our municipality, a FunWalk activity was rendered in cooperation with the parish of St. Joseph the Worker. I participated as part of the medical team assigned at station 2, to give first aid if necessary. The municipal ambulance was also there for any disaster preparedness measure. It started just after the 1st Misa de Gallo for this year. As early as 5am we were already at our assigned area of responsibility. Many Milaorians participated to enhance their health and to live the motto to “Walk together with Christ”.




 


 

          This month was also filled with may celebrations. 1st is our beloved Mayor’s Birthday, LGU Xmas Party and RHU Xmas Party. The BHW also celebrated their own Xmas Party and we helped in packing their gifts from our RHU. We were invited and I gladly accepted the invitation. i even won half sack of rice during the raffle.

















II.    LEARNINGS/INSIGHTS:

          I am getting acquainted with basic medications, their generic and brand name, their actions or indications, dosages, side effects and the like by helping in recording the prescribed medicines by our MHO. The everyday routine with these same drugs ; antibiotic, mucolytic, vitamins, anti-tb, anti- hypertension, maintenances for arthritis & diabetes and vaccines are enhancing my knowledge on how to handle them and basically the facts about drug administrations. This is one of the main skills a nurse needs to focus on, and I am glad this is being handled during our training for RN Heals.


          As a nurse in the RHU as well, my social skills were challenged. Facing different types of people from the municipality is one of the things we should consider professionally. You have to maintain respect and confidence in dealing with them. I learned how to possess control and at the same time be determined in giving necessary nursing measures. Showing compassion and information while being treated. Above all, to have lots and lots of patience to those who are irritable and demanding patients.

III. ISSUES/CONCERNS:

        The center is currently being renovated. Landscaping was already finished.


 

          There is a newly constructed cabinet inside the 2 main rooms. It is still in the process of general cleaning and organizing of things in the RHU.


          The only concern for the month that got my attention was the order of the patients getting in the RHU. There was no organized line for the numerous patients that we have every day. Some are not patient enough to wait and are already arguing that they came first.

IV. RECOMMENDATIONS:

        For the above observation, I highly recommend to have a NUMBERING or a systematic way/ approach of the sequence of assisting patients by the earlier they got in, it will reflect by the number that they were given just as soon as they enter the center. I volunteered to encode the numbers myself and laminate it. We will implement it by the start of the new year 2012.
